How to Register
1. Complete the registration form:
Form 1 (MS Word) for individual registrations and Form 2 (MS Word) for group registrations).
2. Mail (post) a copy of it along with a valid cheque payable to The University of British Columbia (UBC) or fill out the credit card details at the end of the form.
3. If paying by credit card, please fax the completed form to:
School Leadership Centre 604-822-8234, Attention: Dr. John Moss.
Registration confirmation will be made by email once payments have been processed.
Registration CANNOT be confirmed prior to receipt of payment.

Participant List
Every participant will receive a list that will contain name, organization, mailing address, and email address of the conference participants. Your contact information will be added to the participant list unless you opt out on the registration form.
